Monthly institutional restraint and seclusion reports [Massachusetts Department of Mental Health]

Scope and Content: Pursuant to MGLA c 19, the Dept. of Mental Health fulfills its responsibilities toward mentally ill persons in Massachusetts through a comprehensive program of services and facilities.  (It exercised a similar function for the mentally retarded until 1986, when a separate Dept. of Mental Retardation was established.)  In this connection the department was responsible for monitoring the use of restraint and seclusion at facilities under its jurisdiction per MGLA c 123, s 21 (St 1970, c 888, s 4) and regulations.  Monthly institutional restraint and seclusion reports were completed by individual facilities and returned to the central departmental office where they were reviewed and collated.  (They were received at the departmental deputy commissioner’s office as well as by the department’s Quality of Care Review Team.)

Facility survey files [Massachusetts Department of Mental Health]

Scope and Content: Programs and institutions for the mentally ill and retarded of Massachusetts were among the responsibilities successively of the Board of State Charities (St 1863, c 240), the State Board of Health, Lunacy, and Charity (St 1879, c 291), and the State Board of Lunacy and Charity (St 1886, c 101).  They were then the sole responsibility successively of the State Board of Insanity (St 1898, c 433), the Massachusetts Commission on Mental Diseases (St 1916, c 285), the Dept. of Mental Diseases (St 1919, c 350, ss 79-81), and the Dept. of Mental Health (St 1938, c 486)

Scope and Content: The State Board of Insanity, established in 1898, was replaced successively by the Massachusetts Commission on Mental Diseases in 1916 and the Department of Mental Diseases in 1919; that body was renamed the Department of Mental Health in 1938.

Human services resource directory survey forms [Massachusetts Department of Mental Health]

Scope and Content: Pursuant to MGLA c 19, the Dept. of Mental Health fulfills its responsibilities toward mentally ill persons in Massachusetts through a comprehensive program of services and facilities.  (It exercised a similar function for the mentally retarded until 1986, when a separate Dept. of Mental Retardation was established.)  In the early 1970s the department initiated a project to compile comprehensive information on mental health, mental retardation, and other human services available to residents of the thirty-nine mental health catchment areas of the state.  The project, originally planned by the Human Services Resource Information Register Task Force, was coordinated by a resource survey office in  the central office of the department and by the Massachusetts Association for Mental Health, which worked with representatives of catchment areas to collect, edit, and verify information, and finally to compile area directories distributed by area offices.  (Originally an online information and referral system was also planned.)  Human services resource directory survey forms were sent to and collected from human service agencies throughout the state and were used as a basis for preparing area directories.

Deputy commissioner’s special investigation files [Massachusetts Department of Mental Health]

Scope and Content: Pursuant to MGLA c 19, the Dept. of Mental Health fulfills its responsibilities toward mentally ill persons in Massachusetts through a comprehensive program of services and facilities.  (It exercised a similar function for the mentally retarded until 1986, when a separate Dept. of Mental Retardation was established.)  As delegated by the commissioner of mental health (MGLA c 19, s 3), the deputy commissioner was involved in an investigation into the deaths of patients at Metropolitan State Hospital in 1973.  Deputy commissioner’s special investigation files consist of correspondence, memorandums, and reports generated during the investigation.

Commissioner’s files relating to the special legislative commission on mental retardation [Massachusetts Department of Mental Health]

Scope and Content: The Dept. of Mental Health exercised responsibility for mentally retarded persons in Massachusetts (specifically from 1966 through its Division of Mental Retardation) until 1986, when a separate Dept. of Mental Retardation was established. To fulfill the function of providing for the efficient, economical, and humane management of state facilities for the retarded, the department through its commissioner cooperated with the Special Commission on Belchertown State School and Monson State Hospital, established by Resolves 1970, c 19 to study the operation of and conditions at those facilities, creating these files.
